probation until Oct. 20, 2001, due to criminal conviction history.
Shawn McCorkle, Chicago – barber license (006-061959) issued and placed on
probation for three years due to criminal conviction history.
Ross L. Purchas, Justice – barber license (006-061205) placed on indefinite probation
after defaulting on an Illinois Student Assistance loan.
Michael V. Secreto, Bolingbrook – barber license (006-061520) reprimanded and fined
$200 for practicing on a non-renewed license for approximately nine months.
The Comb Hairport, Waukegan – salon license (189-004584) reprimanded and fined
$300 for allowing two unlicensed individuals to practice as barbers and allowing another individual with a non-renewed barber license to practice at the salon.
COSMETOLOGY Merita Caushi, Elmhurst – cosmetologist license (011-256814) issued, suspended for
30 days and fined $500 for practicing for more than four years without a license.
Cheritha J. Ellis, Merrillville, IN – cosmetologist license (011-244248) indefinitely
suspended after defaulting on an Illinois Student Assistance loan.
Exotic Trenz Beauty Salon, Chicago – salon license (189-005168) reprimanded and
fined $175 after the owner practiced as a cosmetologist for approximately eight months without a license.
Cheryl Fletcher, Lake Villa – cosmetologist license (011-256809) issued with
reprimand and fined $175 for practicing approximately nine months without a license.
Candace M. Foys, Elmwood Park – cosmetologist license (011-257001) issued with
reprimand and fined $750 for practicing as a cosmetologist for more than 10 years without a license.
Megan M. Jenkins, Bloomington – cosmetology teacher license (012-006621) issued
with reprimand and fined $300 for practicing as a cosmetology teacher for 1½ years without a license.
Tammi M. Williams, Bolingbrook – cosmetologist license (011-228332) reprimanded
and fined $175 for practicing on a non-renewed license.
