Sub: Prequalification of Vendor for supply of First Aid Box

IDBI Bank Ltd. proposes to invites Bid for supply of First Aid Box & Medicines as per list at various
centers PAN India.
The scope of work would involve
Supply of First Aid Box and Medicines– pan India
Bank would empanel vendors for the pan-India. Vendors satisfying the pre-qualification criteria will be
empanelled for IDBI Bank branches (present and future). Vendors are requested to furnish details
regarding their Company, Organization Chart, Date of Incorporation, Experience etc as indicated in the
application format given in the web site. The application forms can be down loaded from the IDBI
website www.idbi.com from Notices – Tenders.
The application formats duly completed in all respects giving the aforesaid information is required to be
addressed to the officer as indicated in the table below and should be enclosed in a sealed cover super
scribing in the top left hand corner of the cover as "Application for Supply of First Aid Box " and
dropped in tender box kept for the purpose on or before August 06, 2013 by 3:00 pm at the Office
as indicated in Page 2.

Terms and conditions of empanelment
The centers, located in the different States, across pan-India:- Bank would empanel vendors for the pan-India. The supply of First Aid Box for various Branches / Designated Offices proposed in different states across pan-India, where Branches / Offices, located in Rural / Semi- urban / Urban / Metro centers and only those vendors who are capable supplying out in all part of the states need to submit their application to the address given above. Vendors satisfying the pre-qualification criteria will be empanelled for IDBI Bank branches
Qualification / Eligibility Criteria
1. The vendor should have minimum annual turnover of Rs.35.00 lakhs during the three previous financial years ending March 31, 2012 (Copies of audited statements & Income Tax Returns to be enclosed as proof for the same). 2. The vendor should have executed successfully similar works during last 3 years ending March 31, 2012 which should be either of the following:- (a) Three similar completed assignment (at least one of them should be in Central Govt./Central Autonomous Bodies/ Central Public Sector Undertaking/ Financial Institutions/Banks) each costing not less than Rs.15 lakhs (b) Two similar completed assignment costing not less than Rs.20 lakhs (c) One similar completed assignment costing not less than Rs.30 lakhs Similar assignment means "Pharmaceutical distribution / Supply of First Aid Box." __________________

C. Scope of Work:
Quality of First Aid Box:
The boxes should be of standard type MS Power Coated using .24 SWG, wall mounted, with lock and
key facility. The inside items should be placed with proper holding mechanism, so that nothing
should fall down while opening the shutter
D. Time of completion
The delivery period is two month from the date of issue of LOI delivery of the first aid boxes will be reviewed by the Bank after 40 days, and if the supply is not satisfactory then the Bank has right to change the vendor. E. Method of award of work
Tender would be invited only from the pre-qualified vendors. A Pre-qualified vendor who comes out successful in Tendering process will be given the work. Award of additional works would be considered after verifying the successful completion of the works already awarded. F. Submission of Prequalification Documents

F. Instructions to bidders
